Fertility Tests for Women

A female fertility evaluation usually includes the following tests.

• AMH (Anti-Müllerian Hormone) estimates the ovarian reserve.
• FSH and LH assess ovarian function. They are usually drawn on day 2 or 3 of the cycle.
• TSH and prolactin check thyroid function and hormones that affect ovulation.
• A transvaginal ultrasound is used for the antral follicle count and to examine the uterus and ovaries.
• An HSG (hysterosalpingogram) checks whether the fallopian tubes are open. It is done when indicated.

Should Both Partners Get Tested?

Yes, and at the same time rather than one after the other. Male factors contribute to about half of all infertility cases, either alone or together with a female factor. Testing only one partner first often delays an accurate diagnosis by months. Dr. Lekhi recommends that both partners begin testing together once the standard waiting period has passed, rather than waiting on the female partner's results before considering the male partner.

When Should You Get Tested?

Get tested if you have not conceived after 12 months of regular, unprotected intercourse. Get tested after 6 months if the female partner is 35 or older. This timing follows standard recommendations from reproductive medicine bodies. Test sooner, for either partner, if there is already a reason to suspect a problem. These reasons include irregular or absent periods, a diagnosis of PCOS or endometriosis, a past pelvic infection or surgery, a known or suspected varicocele, or a previous abnormal test result that was not followed up.

What Happens After Testing?

Once the results are in, Dr. Lekhi reviews both partners' findings together and explains what they mean for your situation. The next steps depend on what the testing shows. They range from monitoring and lifestyle guidance to ovulation induction or IUI, to IVF. ICSI is recommended only when there is a male factor or another clear indication. It is not a default add-on.
